                                                      HOME HEALTH NURSE (RN)

Our home health care agency is hiring a Home Health Nurse effective immediately. We work with patients of all ages and backgrounds who require daily care in their homes. Most of our patients have physical limitations so we hire the most compassionate and thoughtful talent in the field. You will be responsible for managing care for multiple clients, so time management skills are essential. It is important to provide each patient with the time and attention necessary to meet his or her needs. If you enjoy working one-on-one with patients outside of a clinical setting, then this could be the right career for you!

Home Health Nurse responsibilities:
  • Assess patient's condition during every visit and chart your observations
  • Perform evaluation tasks, including vital signs and medication review
  • Administer medication as prescribed by the patient's Physician
  • Dress or redress open wounds and assess progress of healing
  • Educate patients and their families on proper home health care strategies and procedures
  • Coordinate with Home Health Aids, Physicians and anyone else involved in the patient's care plan
  • Provide palliative care as needed to keep the patient comfortable
  • Make recommendations for devices or tools that might improve the patient's quality of life
  • Listen to the patient and respond to concerns or requests
  • Accurately and timely completion of medical record documentation

Education and Experience Equivalencies:
  • RN License in SD required
  • Applicable experience strongly preferred in home health setting. 
  • Applicants with excellent abilities in communication via email, Word, and medical records preferred.
  • Excellent time management
  • All candidates must have (or qualify to obtain) a current South Dakota driver’s license, proof of insurance and be cleared through the Division of Criminal Investigation.

Physical Requirements:
● Ability to perform duties as outlined above including lifting 10-20 lbs.
● Continual standing and walking during shift.
● Continual talking in person or on the phone during shift.
● Ability to perform duties as outlined above including lifting 10-20 lbs.

                                                      CARE TEAM MANAGER

As a key member of our staff, you will be responsible for the oversight of a team of care providers to ensure an exceptional Stay Graceful client experience! You will manage the scheduling and training process for Stay Graceful care providers and will be accountable for leading the team in a positive, motivating manner with a continuous focus on employee development. As a Care Team Manager, you must exemplify the Stay Graceful goal to provide exceptional in-home care by hiring the best qualified staff, understanding client needs and care plans, and coordinating training and service to deliver exceptional in-home care! 

Responsibilities include:

● Staff Management: 
​
○ Create staffing schedules, including on-call coverage, with the assurance that all shifts are regularly covered. 
○ Manage and train on protocol for absences. 
○ Orient new team members and prepare and submit all HR related forms in a timely manner. 
○ Resolve employee issues or concerns in a professional manner. 
○ Manage disciplinary/termination activities with supervisor support. 
○ Provide backup support for any employee who is absent. 
● Training: 
○ Ensure care provider team reviews and understands policies and procedures. 
○ Select relevant trainings for in-home care providers and implement routine trainings with team. 
○ Oversee competency evaluation of care team members. 
○ Lead by example and maintain consistent accountability for direct reports by training and coaching staff members to provide exceptional customer service. 
● Customer Service: 
○ Meet with new clients and provide individualized client care plans. 
○ Assign care providers to clients who best fit their needs. 
○ Regular review with care providers on completion of care plan and any changes or needs of clients. 
○ Greet care providers, clients and guests, providing exceptional customer service and doing your best to make clients feel comfortable and appreciated! 
○ Answer phones in a friendly manner and assist callers with a variety of questions. 
○ Site visits of current clients for training and support and home visits for team introduction and training for new care providers. 
○ Respond to client and care provider questions and concerns in a timely and professional manner. 
● Client Accounts: 
○ Utilize EMR system to assist with new client accounts, change of address and care plan changes or other pertinent information for current clients. 
● Cleanliness and Maintenance: 
○ Keep the desk area and office clean and orderly. 
○ Notify supervisor of any office or facility needs or technical support requirements. 
● Miscellaneous: 
○ Track Key Performance Indicators such as client visits, cancels, info calls, etc. (weekly, monthly, annually) and relay to supervisor to evaluate Stay Graceful efficacy and reward care provider team for excellence! 


Education and Experience Equivalencies:
  • No degree required. Bachelor's degree preferred. 
    ● Home care or related experience required. 
    ● 1 yr Management experience preferred. 
    ● Customer service experience preferred. 
    ● Applicants with excellent abilities in communication via email, Word, and medical records preferred. All candidates must have (or qualify to obtain) a current South Dakota driver’s license, proof of insurance and be cleared through the Division of Criminal Investigation. 
    ● Must be 18 years of age or older. 
    ● Superior problem solving skills. 
    ● Exceptional leadership and diplomacy skills. 
    ● A passion for caring for others!

                                                    IN-HOME CARE PROVIDER

We expect unfailing honesty, respect for the dignity and individuality of human beings, and a commitment to professional services. With well trained, professional care providers like you, we know we can make be exceptional in caring for clients who want to Stay Graceful!

Responsibilities:
  • Provides direct supervision and daily care of Stay Graceful clients primarily in their home environment.
  • Implements and maintains a professional and caring relationship with all clients; providing a balance of nurturance, limit setting and boundaries while demonstrating understanding of the clients’ needs at all times.
  • Plans and participates in activities with the clients as outlined in the care plan.
  • Attends and actively participates in weekly team meetings, group supervision, and completes in-service hours annually as required by state regulations.
  • Maintains a professional, courteous and helpful relationship with clients family and other primary social supporters.
  • Adheres to Stay Graceful’s Standard Operations Procedures and South Dakota State Regulations/Administrative Rules relating to in-home care.
  • Assures all clients, coworkers and family members are treated equally and with respect.
  • Responsible for the quality of services and completion of necessary in-home care duties and related training which may include:
    • Medication Management
    • CPR training
    • First Aid training
    • Training in life skills and other care techniques and adherence to these techniques
    • Daily care support may include assisting clients in household cleaning, laundry, cooking meals, grocery shopping, communicating with family and friends, and personal grooming.
  • Completes and/or assures completion of necessary documentation and reports by the work day.
  • Knowledgeable and reflect Stay Graceful’s Mission Statement, Core Values and Vision Statement.
  • Each In-Home Care Provider is expected to meet Stay Graceful’s Code of Conduct, Professionalism, and Ethics. 
  • Other duties as assigned.
